Annotation The article examines the history of the medical service of the Transbaikal Cossacks from 1851 to 1920, based on archival materials and periodicals. At the same time, the authors do not consider the provision of assistance to military units, focusing on assistance to the civilian population. The most valuable information is about the reform of the health system. In the article, the events and phenomena from the history of the Cossacks are synchronized with the events of not only regional but also Russian history. The network of medical institutions, personnel, functioning of institutions is considered in detail. The changes that followed the February revolution are of great interest. Zemstvo medicine appeared together with the peasantry. In the spring and summer of 1918, the Bolsheviks temporarily liquidated Transbaikal Cossack army. Cossacks recovered with the establishment of the regime of ataman G. M. Semenov. Due to the unsatisfactory work of the Zemstvo medicine, in the the spring of 1919 the Cossacks formed an independent medical organization. The victory of the Bolsheviks in the civil war in Transbaikal region led to the elimination of the Cossacks. In the autumn of 1920, medical institutions of the Transbaikal Cossack army were finally transferred to the local councils of the Far Eastern Republic. In addition, we study the further fate of some health workers after the liquidation of the Cossack medical service.
